A grant report must be completed and submitted within one month of grant program completion or by the due date listed below. Failure to fully complete this report and return it will prevent the grantee from grant consideration for at least two years.
If available, please attach 1-5 quality digital photographs of the funded program in action. Even though this is not a requirement, CFCI encourages grantees to submit photos. All photos must have signed photo releases of the individuals photographed (click here for a sample form) as well as written consent from the photographer (click here for a sample form). Additionally, by uploading photographs, the organization gives permission to CFCI to use the photo on its website or in print and electronic materials both now and in the future.
Please note that if the deadline falls on a weekend or holiday, evaluations will be accepted the next business day by 5:00 PM Central Time.
